Call us at 907-522-3511 to make your appointment. Please be considerate and give 24 hours notice if you need to cancel or reschedule.
The new patient forms should be completed before your visit. This will save time when you come into the office. It takes approximately 30 minutes to complete all of the forms. Do not leave any blanks. Please use the word "None" not "N/A" on the lines instead of leaving them blank
If your paperwork is completed before you come in, please arrive 10 minutes early for vitals to be completed before the scheduled appointment time.

Please contact your insurance company to find out if chiropractic care is covered. As a courtesy to you, if you call the office and give us all of your insurance information, we might be able to find that out for you.
